世界卫生组织于2024年8月12日发布的报告指出,全球猴痘疫情仍在持续。截至2024年6月30日,116个国家/领土/地区共向WHO报告了99176例实验室确诊病例和208例死亡。猴痘疫情再次构成“国际关注的突发公共卫生事件”。
值得注意的是,Clade Ib型猴痘病毒主要通过性接触传播,主要感染成年人;而近期发现Clade Ia型猴痘病毒则主要影响儿童,并通过多种传播方式传播。
6月新增26个国家报告了934例新增病例和4例死亡。东非的四个新国家(布隆迪、肯尼亚、卢旺达和乌干达)首次报告了猴痘病例,这些国家的病例均为Ⅰ型猴痘病毒。此外,科特迪瓦正在经历与II型猴痘病毒相关的疫情,南非报告了另外两例确诊病例。

猴痘:从非洲森林到全球舞台
猴痘病毒(Mpox virus,MPXV),与天花病毒一样同属于痘病毒科(Poxviridae)的正痘病毒属(Orthopoxvirus)。这种病毒的形态特征鲜明,呈现圆角砖形或卵圆形,大小在200至400纳米之间。猴痘病毒粒子由脂蛋白包膜包裹,内部包含两个蛋白质侧体,以及一个含有大量双链DNA的厚膜核心。

猴痘病毒的核心蛋白包括A29L、A35R、B6R、M1R、H3L等,这些蛋白质在病毒的复制、结构稳定性和与宿主细胞的相互作用中发挥关键作用。病毒的基因组为双链DNA结构,长度约为197kb,基因组末端包含一个相同但方向相反的末端反向重复序列。这一复杂的基因组编码了约190个开放阅读框(ORFs),其中4个位于独特的末端反向重复序列(Terminal Inverted Repeats,TIRs)中。这些重复序列在病毒的基因组稳定性、复制和转录调控中扮演重要角色。

自2022年以来,猴痘疫情在多个国家爆发,不仅局限于传统流行区域,而且在欧洲、美洲、亚洲和中东等地出现病例,这标志着猴痘传播模式的显著变化。截至2024年6月,全球已有116个国家/领土/地区报告了猴痘病例,其中非洲区域报告的病例数最多,其次是美洲和欧洲区域。值得注意的是,刚果民主共和国报告了最多的猴痘病例,而其他国家如美国、巴西、西班牙、法国、哥伦比亚、墨西哥、英国、秘鲁、德国和澳大利亚也有较高的病例数。

Monkeypox virus/MPXV A29L
A29L是细胞内成熟病毒(IMV)表面包膜蛋白(与牛痘病毒VACV,A27L同源)。溶瘤痘苗病毒(VV)属于痘病毒科中一拥有192 kb双螺旋DNA遗传物质的病毒,会在细胞质中繁殖出具有鞘膜的病毒,进而感染其他不同种的哺乳动物细胞及个体.VV中的A27L基因能表达出一14 kDa大小的鞘膜蛋白,此蛋白能与细胞表面上的硫酸乙酰肝素(Heparan Sulfate)作用使病毒跟细胞结合并且可催化细胞融合。

Monkeypox virus/MPXV B6R

Monkeypox virus/MPXV E8L
